Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system. Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Benefits:
Caregivers working 16hrs or more per week are eligible for Full Benefits, 401K and PTO accrual
Mileage Reimbursement:
Currently $0.70/mile (subject to federal rate changes).
Education Assistance:
ASN to BSN assistance (up to $16,000).
🩺 What You’ll Do:
Deliver skilled nursing care as directed by a physician in a home setting.
Assess patient needs and develop individualized care plans.
Monitor patient progress, document services provided, and adapt care as needed.
Educate patients and families on condition management, medications, and recovery.
Collaborate with physicians and interdisciplinary teams to ensure continuity of care.
✅ What You’ll Need:
RN License in Missouri
BLS Certification (American Heart Association)
Valid Driver’s License & Auto Insurance
Preferred: 1 year of experience in home health, acute care, or a related field
